Criteria to apply

There is no fee to apply for the award, and all adult acute hospitals are welcome to participate.

To determine eligibility to apply for the award, organizations will need to have a minimum of 4 "C" level executives complete an 84 item self assessment (in online survey form) that explores the major contributors to hospital labor waste, their most effective solutions and each organization's assessment of its success in optimizing each area. Completing the online survey requires between 12 & 20 minutes per participant.

Failure To Qualify

Organizations who wish to apply are required to achieve a "threshold score" on the self assessment. Should an organization not receive a qualifying score, the award committee will respond both electronically and in writing within 10 business days of receipt of the final survey through the venue of a report that provides:

  • Results of the self assessment (organizational score)
  • A gap assessment between the organizations score and the qualifying score
  • Details on areas of opportunity/deficiency
  • Basic action plans to overcome deficiencies

Qualification To Apply

For organizations who receive a qualifying score on the self assessment, the award committee will respond both electronically and in writing within 10 business days of receipt of the final survey with a phone call to the primary applicant in order to:

  • Provide the formal application
  • Provide the audit checklist that details data requests necessary to validate the results of the self assessment

"Near Miss" Qualification

Organizations who receive a self assessment score within 10% of the qualifying score or who receive results that identify fewer than 5 deficiencies will be contacted and offered the choice to apply with the understanding that:

  • Should they chose to apply, their formal application must include detailed action plans for identified deficiency areas

Reapplying

Organizations who fail to receive a qualifying score are able to reapply no sooner than 90 days from the issuance of the results of their original self assessment. These organizations will be required to begin the application process over again with the completion of a new self assessment. Organizations who receive a "Near Miss" qualification and who fail to provide realistic, valid corrective action plans along with their application and response to audit requests will be required to reapply after a 90 day wait.
